Refund Form

This refund form is used for all authorized Smart Start refunds.

Please carefully review the refund program requirements and information and apply for a security deposit refund.

Submitting a refund form does not guarantee you will receive a full refund.

Refund Eligibility

  • Must have paid a Security Deposit
  • Device must be returned to Smart Start inventory
  • Refund form must be completed and submitted within 30 days of removal or return of the device
  • Valid Email Address and Phone Number must be provided

Disqualifications

  • Security Deposit was not paid or received by Smart Start
  • Device was returned damaged or not functioning
  • Valid Email and Phone Number were not provided

If you feel that you are eligible for a refund:

  1. Complete the refund form
  2. Select form of payment
  3. If you are approved, depending on your payment selection you will receive an email from reward@virtualrewardcenter.com with further instructions or be contacted by a Smart Start representative to complete your refund
  4. If you are not eligible for a refund, you will receive an email with information regarding the disqualification

All refund submissions must contain the above information and be submitted to Smart Start within thirty (30) days of removing or returning the device to receive a refund.
