Smart Start posted a general guide on what to expect for installation day. Now we are going to provide you what you need, state-by-state.

Here’s what you need to bring to your installation appointment by state (not every state is listed, keep checking the blog for more posts on this series.)

Florida

  • A form of photo ID
  • Proof of vehicle registration
  • Proof of vehicle insurance

Georgia

  • A form of photo ID
  • Payment for installation and any applicable fees

Illinois

  • Two forms of ID (one with a picture)
  • Proof of vehicle insurance
  • Proof of residency
  • Payment for installation and any applicable fees

Iowa

  • A form of photo ID
  • Payment for installation and any applicable fees

Kansas

  • A form of photo ID
  • Any relevant court or DMV paperwork
  • Payment for installation and any applicable fees

Kentucky

  • Two forms of ID (one with a picture)
  • Proof of vehicle insurance
  • Proof of residency
  • Payment for installation and any applicable fees

* If you are going to use someone else’s vehicle, you’ll need a letter of permission from the vehicle owner that states it is OK to install the Interlock into their vehicle.

Nebraska

  • Vehicle identification and registration proof
  • Payment for installation and any applicable fees

North Carolina

  • Two forms of ID (one with a picture)
  • A piece of mail for proof of address
  • Any court/DMV paperwork pertaining to your case
  • The vehicle registration
  • Proof of vehicle insurance
  • Payment for installation and any applicable fees

* If you are going to use someone else’s vehicle, you’ll need a letter of permission from the vehicle owner that states it is OK to install the Interlock into their vehicle.

Virginia

  • A form of photo ID
  • Proof of vehicle insurance
  • Any court paperwork relevant to your case
  • Vehicle title and/or registration

* If you need to use someone else’s vehicle, you need to bring the state-approved Owner’s Consent Form, which must be notarized by the owner of the vehicle. Or, the owner of the vehicle can accompany you to your installation appointment.

West Virginia

  • Approval letter from the DUI Interlock Department
  • A copy of the front page of your application
  • Vehicle registration
  • Proof of vehicle insurance

* If you need to use someone else’s vehicle, you can provide page 4 of your application stating that you have permission to install the Interlock on a vehicle that is not registered in your name.
