SERIES: What to Bring to Your Interlock Device Installation Appointment, Part 3
Here is part 3 of what you need to bring to your Interlock Device installation appointment. If you do not see your state in this list, read part 2 here for a list of more states, or a general guide here.
Maine
- Photo ID
- Vehicle insurance
- Proof of vehicle ownership
* If you need to use someone else’s vehicle, you’ll need authorization from the owner to have the Interlock Device installed into their vehicle.
New Hampshire
- Photo ID
- Vehicle insurance
- Proof of vehicle ownership
* If you need to use someone else’s vehicle, you’ll need authorization from the owner to have the Interlock Device installed into their vehicle.
Vermont
- Proof of residency (you can provide a bill for rent or utilities, etc.)
- Photo ID
- Vehicle insurance
Rhode Island
- Photo ID
- Vehicle registration
- Current utility bill with your name and address on the bill
* If you need to use someone else’s vehicle, the owner of the vehicle must accompany you to your installation appointment.
Connecticut
- Proof of residency (bill for rent, utilities, etc.)
- Photo ID
New Jersey
- Photo ID
- Vehicle registration
- Current utility bill with your name and address on it
* If you need to use someone else’s vehicle, the owner of the vehicle must accompany you to your installation appointment.
New York
- Photo ID
- Name and policy number of your vehicle insurance
- The VIN of all vehicles that you own or routinely drive. Then a statement disclosing the names of all other individuals who operate said vehicles.
* If you need to use someone else’s vehicle, you must provide a notarized affidavit from the registered owner of the vehicle.
Pennsylvania
- Two forms of photo ID
- Your eligibility date (on your Restoration Requirements letter)
Massachusetts
- Two forms of ID
- Proof of residency
Delaware
- Proof of vehicle insurance
- Photo ID
- Copy of your completed Ignition Interlock Program application
- Proof of enrollment in a course of instruction and/or rehabilitation as indicated by your court
Maryland
- Driver’s license or valid license number and date of birth.
- Vehicle registration
- Proof of vehicle insurance
In addition to the above requirements for your state, you’ll need to bring payment for installation and any applicable fees.
