South Carolina and Tennessee

  • Cash or debit/credit card payment
  • Photo ID
  • A copy of your Order of Restricted Driver’s License
  • Contact information of your monitoring authority, the point of contact who ordered you on an Ignition Interlock
  • Vehicle registration

*If you need to install an Ignition Interlock in someone else’s vehicle, bring a letter of permission from the registered owner.

Alabama

  • Photo ID
  • Vehicle registration or proof of purchase or ownership

*Bring this waiver form filled out and signed by the vehicle’s owner if you need to use someone else’s vehicle for your Interlock program.

Ohio

Michigan and Wisconsin

  • Cash or debit/credit card payment
  • Driver’s license
  • A copy of your court order

Indiana

  • Photo ID
  • Cash or debit/credit card payment
  • Your vehicle registration
  • Any court documents related to you case involving an Ignition Interlock

Mississippi

  • Your court order, installation form or a note from your monitoring authority
  • Cash or debit/credit card payment
  • Two forms of photo ID
  • Proof of residence

Minnesota

  • Photo ID
  • Cash or debit/credit card payment
  • Proof of residence
  • Any documents related to your case

Missouri

  • Photo ID
  • Cash or debit/credit card payment
  • Proof of residence

Arkansas

  • Cash or debit/credit card payment

Louisiana

  • Photo ID
  • Cash or debit/credit card payment
  • Any documents related to your case
