Adjusting an Ignition Interlock for a Medical Condition

You need to submit any medical considerations that obstruct your ability to perform your Ignition Interlock tests to the Department of Highway Safety and Motor Vehicles’ (DHSMVs’) Medical Review Board.

Your doctor must fill out an Ignition Interlock Medical Evaluation form. You can download and print this form on Smart Start’s site for your doctor to fill out. The form must only be filled out by a board eligible/board certified pulmonologist, or physician whose specialty relates to your condition.

Unable to Take a Breath Test at Service Center

If you arrive at your Ignition Interlock installation appointment before filling out the form and you cannot pass your test, we can print out the medical form for you and reschedule your appointment so you can go through the medical waiver process. There is an additional form that must be completed at the service center with your completed medical waiver form.

Keep in mind that if you are seeking a hardship license, you cannot receive the license until the installation of your Ignition Interlock. If you are seeking permanent reinstatement of your driver’s license, you cannot be reinstated until the installation of your Ignition Interlock.
